This includes support with physical activity, nutrition, medication management, and social connection.You will make a difference bySupporting older people to remain active, connected, and independent through the Seniors Lifestyle Victoria pilot program, delivering a time‑limited intervention funded until June 2028.Be the primary point of contact for the Seniors Lifestyle Victoria program, working collaboratively with participants, their families, and multidisciplinary teams.Develop and implement tailored action plans that address the participant’s goals within the pilot program and promote sustainable improvements in health and wellbeing.Complete assessments relating to client functional outcomes connected to the Seniors Lifestyle Victoria Pilot.Facilitating smooth transitions and facilitating meaningful connections for clients throughout their journey with relevant internal and external services.Ensuring seamless navigation across health and social care systems, facilitating access to appropriate services, and empowering participants to take an active role in their own health journey.To succeed, you will need:Relevant certifications in health related area/s, peer support work, etc. At least 2 years experience working in a community health setting.Experience in motivational interviewing or health coaching techniques.Passion for supporting older persons to build knowledge, skills and confidence to self-manage their health.Ability to work collaboratively in a multi-disciplinary team.Ability to provide culturally inclusive & safe support to a diverse community.Strong technical skills with ability to adapt to new platforms and client management systems.Current Victorian driver’s licence and Working with Children’s Check.We will offer you:Flexibility We work in a flexible, creative and adaptable way to best support our staff and clients.
